When i come to step3 after filling step1 and 2 completely
then when i click submit, it return backs to step 2, omitting all fields and an error message is displayed "Required field is empty".

ensure u are using 240 x 320 resolu... pic for preview it ........... (or select " Automatically generate a preview " option) ......
Are all the fields in step 2 filled in? Ensure they are and you have selected anything in drop down boxes. If you are sure everything is still correct please screenshot the form after you have filled it in for us please.
